Vern's Comments:

The reverse of this little medal has two distinct descriptions. The two authorities disagree on whether it is oak or palm branches and they disagree on whether there is one or two colons in the date. It looks like two colons to me though the second on my example is worn. But neither mention the period after HEALTH.
